Using form ST5 when cancelling Form A restrictions
Providing the right evidence will help us complete your application as quickly as possible.

Last year we introduced a new form ST5 to help customers supply the information required when they want to cancel a Form A (joint proprietor) restriction.

However we still raise a lot of requisitions for this type of application where customers have not used the form and have not supplied sufficient evidence for us to cancel the restriction.

Please consider using form ST5 when you cancel a Form A restriction as it will help you to provide all the evidence we need and should make sure we can complete your application as quickly as possible.
