Fall of a suspended buoy from a workboat crane at a fish farm off the Isle of Muck, Scotland, resulting in serious injuries to a deckhand.

Today, we have published our accident investigation report into the failure of a suspended grid buoy during a lifting operation on board the workboat Annie E on 3 April 2021, causing it to fall and strike the deckhand.

The report, available via this link, contains details of what happened and subsequent actions taken.
