Today, we have published our accident investigation report into the fatal man overboard of the skipper from the fishing vessel Reul A Chuain (OB915) while attempting to recover another crew member from the sea in heavy weather on 24 June 2021.

A safety flyer to the fishing industry summarising the accident and detailing the safety lessons learned has also been produced.
