Lifeboat davit failure during a crew familiarisation drill on a polar research vessel on Loch Buie, Isle of Mull, Scotland.

Today, we have published our accident investigation report into the failure of a davit interlock device on RRS Sir David Attenborough on 4 March 2021, resulting in a lifeboat falling into the sea with three crew on board.
