Today we have published our accident investigation report into the collapse of a crane on board the workboat Carol Anne, resulting in 1 fatality on Loch Spelve, Isle of Mull, Scotland on 30 April 2015.

The report contains details of what happened, subsequent actions taken and recommendations.
